Beginning in October 2023, the City partnered with Sacramento County Department of Homeless Services and Housing (DHSH) to deliver a Street Outreach Program for unhoused individuals in Citrus Heights. The Street Outreach Program is operated by Community HealthWorks, a nonprofit addressing some of the most complicated issues to remove barriers, so all people have an opportunity to be healthy and lead a life of dignity and humanity.
Community HealthWorks provides the City with one full-time Community Health Worker (CHW) who connects unhoused individuals to supportive services, emergency shelter, interim and permanent housing, supplies, and other services to help the individual achieve next steps to becoming housed.
The City also has a strong partnership with local nonprofits, including working with Citrus Heights Homeless Assistance Resource Team (HART). More information can be found on Citrus Heights HART page.
